Advice filling in a PIP form this weekend

What's needed

Money is tight this month and I'd rather ask than go without. There's parking right outside and easy access round the back. Any help at all, even part of it, would make a real difference. Happy to say a proper thank you, and to return the favour when I'm back on my feet. I'm based in Derby, Derbyshire.
